DESCRIPTION:Provides case management for survivors of Hurricane Harvey.

INTAKE PROCEDURE:Call agency and provide contact information. A Case Manager will contact you to schedule an appointment.

FEES:None

DOCUMENTS REQUIRED:Valid ID, Proof of residence during Hurricane Harvey, proof of Hurricane Harvey impact (examples of proof: pictures, FEMA letters, Insurance letters, City or County documents indicating damage to the home) NOTE: additional documents may be requested after initial phone or in-person interview

ELIGIBILITY:Impacted by Hurricane Harvey, resided in coverage area during Hurricane Harvey

HOURS:Mon - Fri 8:00 am - 5:00 pm

LANGUAGES:English, Spanish

AREA SERVED:Aransas, Austin, Bee, Brazoria, Fort Bend, Harris, Montgomery, Nueces, Refugio, San Patricio, and Wharton counties
